One approach is to just allow pilots to write whatever they want to write. That's what people do here anyway.

The US Hawks forum already has lots of "accident reports" - reports of accidents written by pilots scattered throughout the forum. By creating a forum titled "Accident Reports" we would just be helping to organize them better.

The "authority" of any given report (just like any other post) stands on its own merit based on: the experience of the person making the report, their first-hand witnessing of the events, their own credibility on the forum, etc.

Something I whipped up as a start point, please feel free to add your thoughts
An incident report needs to include all the essential information about the accident or near-miss. The report-writing process begins with fact finding and ends with recommendations for preventing future accidents.

Writing any incident report involves four basic steps.

1. Find the Facts, you have to gather and record all the facts.

· Specific location, Weather forecast, Date, time.

· Names of people involved, pilot rating/skills, Any titles, Wire crew, Flight School/management, Instructor, Tandem. Private Party or Recreational

· Events leading up to incident

· Names & witnesses report (witness words)

· Including Pilots name, skill level & report, if possible, Exactly what the Pilot said & did
- (Problem,... report incomplete without Pilots word or Verifying signature)

· Weather, Wind, Environmental conditions, any factors

· Circumstances (Special mission or equipment, Glider, etc.)

· Specific injuries (including part(s) of body injured and nature and extent of injuries)

· Type of treatment for injuries

· Damage to Glider equipment, other property etc.

2. Determine the Sequence
Based on the facts, you should be able to determine the sequence of events. In your report, describe this sequence in detail, including:

· Events leading up to the incident. pilot's Name & flight plan, Any wire Crew, Weather conditions at Launch, etc.?

· Events of the incident. describe the crash, was it a launch, in-flight or Landing, determine length of fall.

· Events immediately following the incident.

Did the Pilot respond : Describe how other Crew responded. Did they call for help, administer first aid, shut down site, move the victim, etc.?

The incident should be described on the report in sufficient detail that any reader can clearly picture what happened. Film related to your report always works. Photos of the accident scene may help.

3. Analyze
Your report should include an in-depth analysis of the causes of the accident. Causes include:

· Primary cause (Based on the common flight rules , your Experience and opinion)

· Secondary causes (Glider or equipment)

· Other contributing factors (Special Notes).

4. Recommend
Recommendations for corrective action might include immediate corrective action as well as long-term corrective actions such as:

· Pilot training, looking at safe flight practices

· Preventive maintenance, keep equipment in good operating condition

· Evaluation flight procedures, including recommendation for a change if need.

Evaluate the School business, Tandem, Instructors or Employees making corrections as need. Suggestions that make Flight safer or rule changes that including administrative & management operation

OVERSIGHT & Record of flight operations: including; Insurance contact info, mandatory reports by any liable party or persons involved acting as a business, school, instruction, Joy Ride or Private party
or... (Was it a normal recreational Pilot & flight)

Finally record of all Crash incidents with a set disciplinary action....
.... which group or individual is causing problems

In all fairness USHPA's crash report web site, isn't that bad.
The USHPA public/private report doesn't show,....
a. Analysis of the causes of the accident (given)
b. Recommendations for corrective action (taken)

Oversight Records
Does USHPA's hold the incident records or crash reports on pilots and are they open for public or member viewing.
(Probably not for legal/liability reasons)

Thanks for the post Eagle.

I wouldn't trust Felipe Amunategui any further than I could throw the entire USHPA Board.

If Felipe Amunategui is the "lead investigator", you can guarantee that everything will be swept under the carpet to protect the guilty. As just one example, it was Felipe Amunategui who demanded that there be no audio recording of my expulsion hearing to cover up their kangaroo court (sorry, no insult intended to any real kangaroos our there).

This demonstrates just how untrustworthy USHPA has become. I was expelled for testifying honestly in a court case, and Felipe Amunategui voted in favor of that expulsion. Yet he's the "lead investigator" on their supposedly "secret" accident reporting committee?

     USHPA's Motto:    Speak up and be expelled   (but trust us anyway)

Does the FAA need any more reason to not trust USHPA's "self regulation" of hang gliding and paragliding?

Trust is very important. It has to be earned. The modern USHPA leadership has destroyed any trust that we (or anyone else) can have in that organization.

How can they legitimately claim they have a confidential accident reporting system when they just expelled someone for speaking out about safety abuses at Torrey?

How can "Dr. Felipe Amunategui" be trusted to lead an accident reporting system when he participated in expelling someone for testifying in a case where a young woman was injured by one of USHPA's most valuable conce$$ionaires?

Consider the merit of mess. Mess lets visitors meet things they may not have met before. Thinking starters.
Consider placing a topic thread on some HG incident. Invite the mess. Let guesses be posted. Let questions be posted. Let speculation occur. Discuss things surrounding what concerns pilots and thinkers about an "incident". Let evidence come in the scene as it might. Let an incident spark meditation over safety matters. Let "causes" be traced to eating too large of a breakfast, etc. Even Butterfly Effect. Let it all out. New projects will be seeded. Research committees may be established. Let theories be presented. Let various perspectives be aired. Don't close the file or folder on an incident; there may be more to learn about hang gliding involvement from this or that incident. Link the discussion thread in the master Index of Incidents; link other places the incident is being discussed; link also "official reports" concerning the incident. Realize that "official" does not mean somebody has the right appraisal of an incident; an "official" report carries the bias and perspective of a certain group of people, not all people, and not necessarily all those who may see something better than the "official"s. Deriving value from an incident can come from many other teases than "official" reports. Waiting for "official reports", then simply accepting the "official report", and then perhaps ending the attention on the matter: Such risks the loss of community learning processes. Let everyone have an opinion, even if the opinion is from some apparently distant perspective. What is the impact of neglecting to make peace with one's spouse on incidents? Etc. Official reports may miss important matters.

Consider the merit of mess ...

I agree with every line in Joe's post. There is certainly a place for "official" reports where witnesses and facts have been sifted, analyzed, credited (or discredited) and woven into an easily digestible form. But that "easily digestible form" is bound to miss things and bound to be colored by those who want to produce a result with their particular slant. The "mess" is the reality, and it should be available for all to read and evaluate on their own.

USHPA's tight control of all communication regarding accidents has only resulted in more accidents. My own report to USHPA of the 2010 student landing near-miss at Torrey foreshadowed the Hamby accident at Torrey in 2011 that ended up contributing to USHPA's uninsurability. Yet that report was never published by USHPA or distributed to USHPA members. If it had been, then Shannon's accident might not have happened, and USHPA might not have needed to collect 2 million dollars from its members ... to insure themselves.

We can do better.
